Upon signing & submitting this membership application, I hear by waive and release all other T.I. Bassmasters members, hosts, sponsors and tournament officials from all claims of death , personal injury , accidents , theft , and damage to personal property in connection with these association’s tournament events. I assume full responsibility for my health and safe being. I assume responsibility for all of my equipment on + off the water during these tournaments. I have sufficient health, accident and liability insurance and if I have no insurance , I represent that I am capable of paying for any and all such expenses and liability . I understand the T.I. Bassmasters officials may refuse my application + refund my entry fee for any reason. I give T.I. Bassmasters officials permission to use my name , photograph and film footage of me if it chooses to do so. I have read the rules and regulations of this association and agree to abide by all of them.
